Retatrutide: what a record 28.3% weight loss does and does not show

Eli Lilly's triple agonist produced the largest weight loss of any major obesity trial. The efficacy is clear; the tolerability, the heart data and the timeline need more care.

GLP-1GIPGlucagon

Retatrutide is the most efficacious obesity drug yet tested at Phase 3 scale. It is also the clearest case in the pipeline of efficacy being bought with tolerability. On our GLP-1 Performance Index it ranks second, at 6.6, behind tirzepatide at 8.8.

Mechanism

Semaglutide activates one incretin receptor (GLP-1). Tirzepatide adds GIP. Retatrutide adds a third, the glucagon receptor. GLP-1 and GIP drive appetite suppression; glucagon agonism is thought to raise energy expenditure and reduce liver fat. The combination works on both intake and expenditure, which is the main hypothesis for its larger effect.

The TRIUMPH results

TRIUMPH-1 (n=2,339, adults with obesity and no diabetes) reported on May 21, 2026: 28.3% mean weight loss at 80 weeks on the 12 mg dose (efficacy estimand), 25.0% on a treatment-regimen basis, against 2.2% on placebo. In a pre-specified extension for participants with BMI of 35 or more, the top-dose arm reached about 30% at 104 weeks.

TRIUMPH-2 (type 2 diabetes) showed 20.8% at the top dose; diabetic populations consistently lose less on incretin drugs. TRIUMPH-3 (severe obesity with cardiovascular disease) showed 22.6%, but its cardiovascular endpoints were not statistically significant (MACE-5 hazard ratio 0.82, MACE-3 1.12, wide confidence intervals). That result is inconclusive rather than negative.

Context. Main-trial weight loss: tirzepatide 22.5% (SURMOUNT-1), semaglutide 16.9% (STEP 1), retatrutide 28.3% (TRIUMPH-1). Separate trials with different lengths and populations.

Tolerability

Nausea affected 42.4% on the top dose vs 14.8% on placebo. The distinctive signal is dysesthesia, an abnormal skin sensation, reported by 12.5% on 12 mg vs 0.9% on placebo; the mechanism is not yet understood. Discontinuation for adverse events reached 11.3% vs 4.9% on placebo, nearly double tirzepatide's rate. That is what costs retatrutide the top position on our tolerability-adjusted index.

Timeline

Lilly plans to submit to the FDA in Q1 2027 and is completing chemistry, manufacturing and controls work. A standard review implies a decision around early 2028; that is our estimate, not company guidance. Products sold online as "research peptide" retatrutide are unregulated and not the material tested in TRIUMPH.

What to watch

Dose selection at launch, whether lower doses (9 mg delivered 25.9%) offer a better efficacy-to-tolerability ratio, the full characterisation of dysesthesia, and the cardiovascular outcome data that will determine how payers position it. Full data in the retatrutide profile.
