Setmelanotide
A melanocortin-4 receptor agonist for rare genetic forms of severe obesity.
Effects studied or claimed
What it is used or promoted for, and where each effect has actually been seen
- Weight loss in POMC, PCSK1 or LEPR deficiencyFDA approved
- Weight loss in Bardet-Biedl syndromeFDA approved
How it is thought to work
Setmelanotide activates the MC4 receptor, a key appetite and energy balance pathway that is impaired in certain genetic conditions.
What the evidence shows
Pivotal trials in very small groups of people with specific genetic defects showed large reductions in weight and hunger, which is not seen in common obesity.
Safety and unknowns
Skin darkening, injection site reactions and nausea are common. Spontaneous erections and sexual adverse effects have been reported.
Regulatory status
FDA-approved as Imcivree in 2020, with an expanded indication for Bardet-Biedl syndrome in 2022.
Regulatory status changes quickly. This entry was last reviewed on 27 October 2026. Confirm current status with the FDA before relying on it.
